
Case Manager- Keeping Kids in School Santa Rosa, CA
Job description
As the Case Manager on the team you will provide direct support to youth and families in Sonoma County that are experiencing barriers to school attendance and engagement. You will provide culturally competent and sensitive services including: targeted case management services, skill building support, and connection to school and community resources.

- Conduct assessments to identify needs, barriers, and strengths to establish individualized goals and action plans with clients, caregivers, and school teams
- Facilitate monthly action plan review meetings with clients, families, school teams, and other participating members
- Provide youth and family with linkage to school and community resources to meet their needs and establish sustainable solutions
- Integrate into assigned school communities by working with administration and school intervention teams, attending multidisciplinary meetings, and participating in other school events
- Develop and maintain healthy partnerships with a variety of mental health, school, and community providers
- Provide technical assistance in the form of school or district wide interventions and participating in school or district wide events
- Advocate for creative problem solving to address underlying student/family barriers to success in school
- Make efforts to understand the history of Black, Indigenous, People of Color (BIPOC) in the United States and further Diversity, Equity, and Inclusion

Experience/education requirements:
- Bachelor’s degree OR
- Associate’s degree or equivalent + 1 year of applicable work experience OR
- High school diploma/GED + 2 years of applicable work experience
- Applicable experience includes any paid or unpaid experience working with youth or families, or working in the social services field
- Own vehicle, valid California driver's license, and insurability with Seneca's automobile policy
- Must be at least 21 years of age
- Clearance of TB test, fingerprints and any other mandatory state/federal requirements
- Proof of COVID-19 vaccination
- Bilingual in Spanish and English
- Two or more years of direct work experience in community or school-based services youth and families
- Strong organizational skills as demonstrated by the ability to effectively manage multiple tasks
- Full-time.
- Mon - Fri; 8AM - 4PM
- Flexible schedule, ability to work some evenings and weekend hours as needed
- Starting at $22.00 - $24.00 per hour, actual salary is dependent on creditable experience above the minimum qualifications for the role
